Each year, parents will need to complete a medical and release form for children from birth to grade 5. You can fill it out, print and turn into the Children’s Ministry staff. CHILDREN’S MEDICAL RELEASE FORM If you’re new to BCC Children’s Ministry, please take the time to fill out our information form so that we can place your children in the appropriate grade or class, and be able to contact you with updates, our monthly newsletter and upcoming events. CHILDREN’S FAMILY INFORMATION FORM If you have a child with special needs, please fill out this intake form to give us a quick snapshot of your child. We will then connect you with our Director of Special Needs ministry. CHILDREN’S INTAKE FORM If you have a student in grades 6-12 who is or will be attending Hang Time or Chill Out, please fill out this interactive Medical Release form.
